Does my insurance require a referral?

Most HMO medical and DHMO Dental insurances do require referrals, please call your insurance to make sure that this does not pertain to you.



Do I need to provide my medical or dental insurance?​

Please provide your medical and dental insurance, now days many insurances cover third molar extractions under medical. We would be happy to check that for you!
